This applies whether you make a single donation, or a number of smaller donations, which total at least €250, in one calendar year.

FLAC can claim back an additional €112.32 from Revenue, bringing the total value of your donation up to €362.32. It could be worth an extra 45% of your original donation.

Whether you are PAYE or Self-Assessed, FLAC can claim tax relief on your donation.

To reclaim this extra tax back for FLAC, complete this CHY 3 form. It is an enduring certificate, which is valid for 5 years, unless you cancel it before the end of that period.

Return the original cert including your PPS number and signature, to:

The form does not commit you to donating each year, it simply enables FLAC to obtain tax relief if you donate €250+ in any of the five years.
